Motor Vehicle Traffic Accident
SERGIO OSMEÑA - Be informed that at about 9:30 o’clock in
the evening more or less of June 4, 2017, one (1) Dondon Alfar reported to
Sergio Osmeña MPS that there was a vehicular accident transpired at National
Highway of Brgy. Poblacion Alto, Sergio Osmena, Z.N particularly at SB session
hall involving red Honda XRM 110 without plate number attached driven by RALPH VASQUEZ y PALUBON, male, 21 years
old, single, Filipino and a resident of Purok Dapdap, Brgy Poblacion Alto,
Sergio Osmena, Z.N and a pedestrian identified as ELENA MOAY y SAVILLINO, female, 30 years old, single, Filipino and
a resident of Brgy Poblacion Alto , Sergio Osmena, Z.N. Acted on the said
report, PNP personnel of Sergio Osmeña MPS proceeded to the area to verify the
veracity of the said report. Initial investigation conducted by the IOC,
disclosed that at about 9:25 o’clock in the evening more or less of June 4,
2017, the driver of said motorcycle traversing the National Highway of
Poblacion Alto, Sergio Osmena, Z.Nwithout headlight was accidentally
bumped/sideswiped the pedestrian who was walking beside the road of said highway
that resulted to incur a minor injuries in the different parts of her body, The
victim was brought to Sergio Osmeña Municipal Hospital for medical treatment.
The driver was temporarily detained at Sergio Osmeña MPS after he fails to
present documents as well as his driven mc for proper disposition. For

Spot Report reAlleged Thievery Incident
DIPOLOG CITY - Be informed
that at about 8:00 o’clock in the evening of June 4, 2017, Dipolog CPS received
a call from Aleson Police Outpost, located at Sta. Filomena, Dipolog City
informing that there was alleged thievery incident that transpired at Leyton
Ville Subdivision located at Purok 2, Sta. Filomena, Dipolog City. Acted on the
said report, PNP personnel of Dipolog CPS proceeded to the area of incident to
verify the veracity and to conduct initial investigation thereat. Initial
investigation conducted disclosed that the unit number 11 of the above-named
subdivision which was rented by Mr. and
Mrs. OLUSEGUN SALAMI y OLUSESAN, 56 years old, a resident of said place was
entered by unidentified suspect, thru the sliding window of the servant room at
the second floor, at the back portion of the house and once entered proceeded
at the ground floor and took and carted away
the following: brown Prada Sling bag containing silver Apple IPAD,
silver IPhone 5, wedding ring with one (1) big diamond and three (3) small
diamonds, necklace with three pendant weighing 15 grams, one (1) ring for men
which weigh is 12 grams, two (2) heart shape rings, one (1) flower shape ring,
one (1) bracelet weighing 12 grams, driver’s license and International Driver’s
License under the name of Arlene Padao Salami, regions bank card, debit and
credit card, Western Union Card, USAA Debit and Credit Card, Pag-ibig Payment
Receipt, Gucci wallet, Social Security Card of Arlene Padao Salami, Rory
Tristan Salami, Reagan Tracey Salami, duplicated key of the car, BPI cards,
duplicated keys both of main and back door of the house and cash money amount
to Five Thousand Pesos (P5,000.00). After the incident, the suspect(s) exited
where he gains his entrance. Incident discovered at around 8:40 o’clock in the
evening of the same date. PNP personnel of Dipolog CPS still conducting

Fire Incident
DIPOLOG CITY - Be informed
that at about 5:00 o’clock in the afternoon of June 4, 2017, Dipolog CPS
received a call from a concerned citizen that there was alleged conflagration
incident that transpired at Motorpol, Minaog, Dipolog City. Upon the receipt of
the said report, the Bureau of Fire Protection and Dipolog CPS proceeded to the
area of incident for verification and investigation. Initial investigation
disclosed that the Carpentry Division of the City Government of the
above-mentioned place wherein trashes, scraped woods and sawdust were caught by
fire, based on the investigation conducted by the BFP. Further, the incident
was started from the open burning for few days, and due to unsecured open
burning, it was led into escalation of fire into nearby trashes or stocked
sawdust in the area. One (1) unit Motor Vehicle with Plate Number JAN 385 six
bundle of PE pipe were damaged by the said fire incident. Fire started at
around 4:18 o’clock in the afternoon of June 4, 2017 and declared fire out at
about 4:29 o’clock in the afternoon of same date with an estimated cost of
damaged amounting to more or less Nine thousand pesos (P9,000.00) and no

Spot Report reAlleged Fire Incident
DIPOLOG CITY - Be informed
that at about 6:00 o’clock in the evening of June 4, 2017, Dipolog CPs received
a cellphone call from a concerned citizen that there was an alleged
Conflagration Incident that transpired at Padre Ramon Street, Estaka, Dipolog
City particularly in front of Holy Rosary Cathedral. Acted on the said report,
PNP personnel and BFP of Dipolog City immediately responded to the call and
proceeded to the area of incident to verify the veracity of the report and to
conduct investigation thereat. Initial investigation conducted disclosed that
one (1) unit of Nissan Patrol bearing Plate Number GEM 722, owned by Atty. John
Elumba, 46 years old, married and a resident of Esperanza Village, Turno,
Dipolog City, was conflagrated while parked at the said place. Further,
investigation conducted by Bureau of Fire Protection, the possible cause of the
incident was eliminated into electrical short due to lose contact. Fire started
at around 7:35 o’clock in the evening of June 4, 2017 and declared fired out
around 7:45 o’clock in the evening of the same date with estimated cost of

Recovered Illegal Drugs inside Zamboanga del Norte
Correctional and Rehabilitation Center.
MANUKAN - Be informed that at about
4:30 o’clock in the evening of June 1, 2017, Manukan MPS received a cellular
phone call from the personnel of Zamboanga del Norte Correctional and Rehabilitation
Center, Brgy. Don Jose Aguirre, Manukan, ZN informing said office that they
recovered pieces of shabu inside jail vicinity. Acted on the said report, PNP
personnel of Manukan MPS responded to the said call and proceeded to the above-mention
place purposely to verify the veracity of the report. Initial investigation
conducted by the responding team disclosed that at around 3:30 o’clock in the
afternoon more or less June 1, 2017, two unidentified persons on board with
yellow Honda Beat motorcycle, approached one Rex Ivan Ocum y Abing, 14 years old, Grade 7 student, and a
resident of Brgy. Don Jose Aguirre, Manukan, ZN, who were herding their goat
beside the National Highway of said place, outside the vicinity of Provincial
Jail. Accordingly, one of the two
unidentified persons, offer him one-hundred-peso bill to get something
inside the Jail, which being place near the yellow flowers. Rex Ivan Ocum
refuse to do so. After which he hurriedly returns to their house and told his
neighbors about it. At that time, Maximo
Jatico y Totao, 49 years old, married, resident of Brgy. Serongan, Manukan,
ZN, Prison Guard of the said Jail taking his snack at the store outside the
said facility heard the story of Rex Ivan, he immediately went inside the
Provincial Jail and inform his co-workers regarding the said matter. After
which they conducted search on the describe area and at around 4:20 o’clock in
the afternoon same date Mary Jane Urgel
y Ansilig, 44 years old, married, resident of Brgy. Gupot, Manukan ZN, saw
a cut piece of cigarette case at the grassy portion beside the pathway inside
the vicinity of Provincial Jail and when she opens it they discovered 3 small
pieces of heat sealed transparent plastic sachet containing white crystalline
granules believed to be shabu and 3 small cut pieces of unused rolled aluminum
foil. Further investigation revealed that the above mentioned shabu and foil
was coming from the unidentified inmates of said jail facility and was brought
by their visiting relatives believe was left over where it was recovered before
going outside the vicinity of the jail facility. Recovered items is now under
the custody of Manukan MPS for proper disposition and for turn-over to Crime
Laboratory for examination. The three (3) pcs

Found Dead Body
SERGIO OSMÑA - Be informed
that at about 10:05 o’clock in the morning of June 5, 2017, one (1) Joseph Agad y Bustaliño, 46 years old,
married, Filipino and a resident of Purok 2, Brgy Nebo, Sergio Osmeña, Z.N
reported to Sergio Osmeña MPS that his daughter was found dead at the creek
situated at Purok 2, Brgy Nebo, Sergio Osmeña, ZN. Acted on the said report, PNP
personnel of Sergio Osmeña MPS proceeded to the area of incident to verify the
veracity of the report and to investigate thereat. Initial investigation
conducted by the Investigator On-Case disclosed that at about 5:30 o’clock in
the afternoon of June 4, 2017, One (1) Wilma
Castillon y Agad, female, 42 years old, married, Filipino and a resident of
Purok 2, Brgy Nebo, Sergio Osmeña, Z.N found at the creek near her rice field,
the dead body of her niece, named Maria
Cristina Agad y Obo, female, 6 years old, a resident of said place. According
to her (Wilma), when she visited and was roaming around herrice place, which
the creek was located adjacent to her rice field, she personally saw the victim
(Maria) lying down on the water, lifeless, with an injuryon her right face that
prompted her to informed the parents of the victim. Sergio Osmeña MPS are
conducting thorough investigation to determine the motive of the incident and

Arrest of Wanted Person
DIPOLOG CITY - Be informed
that at about 5:00 o’clock in the morning of June 5, 2017, personnel of La
Libertad MPS led by PINSP RONALD ALLAN E TINGAS together with CIDG personnel
led by PCI SAMSON PASCUAL, conducted manhunt operation in Sunset Boulevard,
Central Barangay, Dipolog City and arrested one ANJANETTE H SULIT, 38 years old, married, a former Municipal Mayor of
La Libertad and a resident of Barangay Poblacion La Libertad, ZN by virtue of a
Warrant of Arrest issued by RODOLFO A. PONFERRADA, Chairperson of the SANDIGANBAYAN,
Sixth Division under Criminal Case No. SB-17-CRM-0465 for “Violation of Article 218” (Failure
of accountable officer to render accounts)of the Revised Penal Code dated
March 7, 2017 with bailbond of six thousand (P6,000.00) pesos. Subject person
temporarily detained at La Libertad MPS. For information.
Campaign against Illegal Drugs
LA LIBERTAD - Be informed
that at about 6:30 o’clock in the mornig of June 5, 2017, PNP personnel
responded a call from the Brgy Captain of Sta. Catalina, which a certain PABLITO JR.
GALANIDO y SENCIO aka “Jun-jun”, 32 years old, married, and a
resident ofsaid place, got wild and
was uncontrolable. The suspect attacked the captain after he drew his 14 inches
knife, fortunately he was avail to evade the suspect’s attacked. Acted on the
said report, PNP personnel of La Libertad MPS proceeded to the said area of
incident wherein the responding team tried to convince the suspect, who was
uncontrollable holding his knife, to peacefully surrendered. After a few
minutes of talking and distructing the suspects, other responding team, grabbed
and succesfully arrested the suspect. Further, after the arrest, a body search
was made and two (2) pcs of sachets of shabu was found inside his pocket. This
searched was witness by the Brgy. Captain Aurio Jauculan. Recovered from his
possession and control were one (1) 14 inches knife and two (2) sachets
containing white crystalline granules

informed that at about 3:15 o’clock in the afternoon of June 5, 2017, PNP
personnel of Labason MPS conducted manhunt operation at Brgy Lapatan, Labason,

BOLONGON, 45 years old, male, single, professional driver by occupation and
a resident of said place by virtue of Warrant of Arrest issued by Hon Judge
Oscar D Tomarong of RTC Branch 28, Liloy, ZN under Criminal Case No L-003131

Criminal Case No. S-3620 dated November 16, 2004 with no bail recommended; for
the crime of “Rebellion” issued by
Presiding Judge Reymar L. Lacaya, of the same court docketed under Criminal
