Accomplishment Report on the Execution of
Search Warrant Nr 05-2016
(Violation of R.A.10591)
DIPOLOG
CITY - On October 7, 2016 at about 5:00 o’clock in the morning, personnel from
ZNPPSC led by PSI PAULO Q BENITO executed the Search Warrant NR 05-2016 for
Violation of RA 10591 issued by Presiding Judge Victoriano DL Lacaya Jr of RTC,
9th Judicial Region, Branch 9, Dipolog City to the house of live-in partners
identified as BIENVINIDO BELLENO AGUSTIN, 35 years old, single and WILME
GALLEMIT GESTOPA, 35 years old, married in Sitio Mahayag, Brgy Aseniero,
Dapitan City. That during the course of search with the investigators of
Dapitan CPS led by PO2 Nick Quimiguing, the team found and recovered personal
properties which are the subject of the Search Warrant, to wit: a. one (1) unit
cal 9mm Uzi sub-machine gun with Serial Nr 58441 with one (1) steel magazine
inserted with six (6) live ammo; and b. one (1) unit cal. 45 Caspian with
Serial Nr 413975 with one (1) steel magazine inserted with three (3) live ammo.
Also recovered in plain view, one (1) pc rectangular transparent sachet
containing white crystalline granules believed to be Shabu. Inventory and
marking of evidence was witnessed by Hon Joselito C Paradiang and Hon Ronilo B
Dawat, both Brgy Councilors of Brgy Aseniero, Dapitan City.The arrested
suspects and the confiscated items were brought to Dapitan CPS for proper

Report on Motor Vehicle Traffic Accident
JOSE
DALMAN - At about 4:35 o,clock in the
afternoon of October 7, 2016, a motor vehicle traffic accident transpired at the
national highway of Brgy Poblacion, Jose Dalman, Zamboanga Del Norte. Initial
investigation conducted disclosed that motorcycle involved were: MC1- Black XRM
motorcycle bearing plate number 7717 VW, engine number xrm17e066440, chassis
number xrm17066467 owned and driven by MICHAEL IVAN ASTILLERO, 24 years old, single
and resident of Poblacion North, Polanco, Zamboanga Del Norte; and MC2 - Black
Honda Wave bearing plate number JU 3160, driven by ROLAND PARTOSA y LONTAYAO,
legal age, married and resident of Brgy Tamil, Jose Dalman, Zamboanga Del Norte
with backrider identified as CHRISTIAN JAIL SULTAN y LUNJAS, 11 years old and resident of Brgy Poblacion, Jose Dalman,
Zamboanga Del Norte. Further investigation revealed that the two MCs were
traveling in the same direction, from Sindangan Municipality going to Polanco,
ZN wherein MC1 was ahead of MC2 and upon reaching at the place of incident, MC2
(driver believed to be under the influence of liquor) accidentally bumped MC1.
The latter sustained injury in his hand while the driver of MC2 and his
backrider sustained serious injuries and were brought to Manukan Community
Hospital onboard PNP Marked Vehicle (hilux) for their immediate medical treatment.
The damaged motorcycles involved are now impounded at Jose Dalman MPS for proper

Spot Report on Fire Incident
DAPITAN
CITY - On October 7, 2016 at about 9:15 o’clock in the evening, a concerned
citizen informed Dapitan CPS thru cellphone call that the abandoned PNP-LGU
outpost located beside Polo Bridge was razed by fire. Immediately personnel of
Dapitan CPS informed the Bureau of Fire Protection (BFP) of Dapitan City and
responded to the above-mentioned place. Joint investigation conducted by the
said responding Team disclosed that based on the CCTV footage gathered near the
place of incident, said outpost was intentionally burned by a certain RAMIL
CALLO y VIDAD, male, more or less 32 years old, a resident of Dapitan City and
believed to be mentally-ill. Subject person was brought to Dapitan CPS for

Report on Stabbing Incident
DIPOLOG
CITY - At about 1:10 o’clock early dawn of October 8, 2016, a concerned citizen
reported to Dipolog CPS that a stabbing incident transpired at Central
Barangay, Dipolog City. Immediately, elements of Dipolog CPS proceeded to the
above-mentioned place to verify the veracity of the report. Initial investigation
conducted disclosed that the two victims were identified as: JOVANE PARPAN y TORRES,
27 years old, married and a resident of Ranillo Street, Santa Cruz, Central
Barangay, Dipolog City; and REY IAN SUMOSOT, 29 years old, single and a
resident of Pulpogan, Consolacion, Cebu
City while the suspect was identified as NIEL TRISTAN JOSEPH, 17 years old,
single and a resident of Barrio Lapuk, Brgy
Barra, Dipolog City. Further investigation conducted disclosed that while the two
(2) victims together with other peers were having a drinking session outside
the FAT CAT Resto Bar, Dipolog City, the herein suspect got angry because his
girlfriend was with the group of the victims. During their confrontation,
victim Rey Ian pacified but the suspect drew his batangas knife measuring 8.5
inches long including its handle and stabbed victim Rey Ian hitting in the left
side of his abdomen. The suspect then turned his attention to victim Jovane and
stabbed him twice hitting him in the left side of body and stomach. During the
incident the herein reportee was able to wrestle the knife of the victim turned
over to Dipolog CPS while the suspect ran away from the crime scene. PNP personnel
of Dipolog CPS responded to apprehend the suspect but were able to elude
arrest. Both victims were brought to Corazon Aquino Hospital but later were
transferred to Zanorte Medical Center and was attended by DR. MASCARIÑAS due to
their stabbed wounds. For information.
Report on Arrest of Wanted Person
SINDANGAN
- At about 4:40o’clock in the afternoon of October 8, 2016, elements of
Sindangan MPS conducted manhunt operations at Sitio Layasan, Barangay Bago, Sindangan,
Zamboanga del Norte that resulted to the arrest of one JERRY CADUNGOG y
ENOJARDO, 41 years old, male, married, Filipino, farmer by occupation and a
resident of Bgry. Pangalalan, Sindangan, ZN by virtue of warrant of arrest issued
by Nimfa B. Carreon- Padao, Presiding Judge of Third Municipal Circuit Trial
Court, Ninth Judicial Region, Sindangan, Zamboanga del Norte docketed under
Criminal Case number 12654 for the crime of “Concubinage” dated August 23, 2016
with a recommended bail bond of Php10,000. Subject person now detained at
Sindangan MPS for proper disposition. For information.
Spot Report on Shooting Incident
MANUKAN
- At about 4:35 o’clock in the afternoon of October 9, 2016 a cellphone call
received by Manukan MPS informing that there was a shooting incident transpired
at Brgy. Don Jose Aguire, Manukan, ZN. Team led by SPO4 Tito C. Barrios, MESPO
responded to the area. The victim
identified as one JUNRILL CONSOLAR AQUINO “aka” Taleo, 26 years old, single,
resident of brgy. Don Jose Aguire, Manukan, ZN a drug Surrenderer (PUSHER) and
currently under Top five (5) in Drugs watch list of Manukan MPS. Initial
investigation disclosed that while the victim was seating at the bench outside
the house of Laarni Aquino at the intersection of brgy Don Jose Aguirre near national
highway leading to Brgy. Gupot, Manukan, ZN. Suddenly two unidentified person
wearing bull cap, face mask and sunglasses riding a red Honda XRM with no plate
number attached arrived at the scene and the back rider approached the victim
and without apparent reason pulled a short firearm and shot the victim three
times hitting on the different parts of his body that resulted to his
instantaneous death then the suspects fled towards Roxas Municipality. Motive
of the incident believed to be drug related. Follow up Investigation is still
